DPWH DEPARTMENT ORDER NO. 13, s. 1994 SUSPENSION/BLACKLISTING OF A.S. MUÑOZ CONSTRUCTION January 06, 1994

In view of the admission by Mr. Agustin S. Muñoz that he unilaterally enlisted the corporate name of SRM Construction to ensure A.S. Muñoz Construction’s qualification in the bidding for the Construction of De Maria-Silvino Lobos Feeder Road (Phase II), SIRDP, Northern Samar, the said A.S. Muñoz Construction and its owner/proprietor are hereby suspended/blacklisted from participating in any bidding or entering into any contract with the DPWH and its line agencies until he has fully paid the advance payment made on the terminated contract but in no case less than a period of one (1) year effective this date pursuant to Section 1B 10.5.1(a) (1) of the Implementing Rules and Regulations of P.D. 1549, as amended.
