DOA ANIMAL INDUSTRY MEMORANDUM CIRCULAR 6 s. 1993 SCHEDULE OF FEES AND CHARGES FOR THE SERVICES RENDERED BY THE ANIMAL FEEDS STANDARD DIVISION OF THE BUREAU OF ANIMAL INDUSTRY March 29, 1993

Pursuant to the following Ministry Order No. 6-83 of then Ministry of Finance on the Rules and Regulations Implementing Batas Pambansa Blg. 324 dated August 29, 1983; Joint Department of Agriculture and Department of Health Administrative Order Nos. 33-IIIA, 138-100; Department of Agriculture Administrative Order Nos. 25 and 37; BAI-AIMC No. 6 all series of 1991, the following schedule of fees and charges for services rendered by the Animal Feeds Standard Division of the Bureau of Animal Industry is hereby prescribed for the information and guidance of all concerned.

4.1 Commercial Mixed Feed/Feed Ingredient Manufacturers/Importers/Indentors/Feeds and Feedstuff Retailers/Feedstuff Suppliers/Feed Distributors/Non-Commercial Manufacturers Registration Fees, and Inspection Fees on Commercial Mixed Feeds and Feed Ingredients:
4.1.1 Within fifteen (15) days after the due date - 25% of the amount due
4.2.2 Over fifteen (15) days but not more than thirty (30) days after due date - 50% of the amount due
4.3.3 Over thirty (30) days but not more than forty-five (45) days after due date - 75% of the amount due
4.4.4 Over forty-five (45) days after due date - 100% of the amount due
4.2 Veterinary Drug and Product Establishments
4.2.1 Renewal of L.T.O. - Fifty percent (50%) surcharges after due date.
		Due date: Initial approval date of L.T.O.
SECTION 5. Surcharges for Late Payments of VDAPE/VDAPO License to Operate and Certificate of Product Registration After Due Date
5.1 50% of the amount due
